I hereby grant permission to Soft Corner Midwifery to use photographs and audio of me taken for use in promotional materials and publications both online and in print or for any other lawful purpose. I understand that this photo project exists as a fundraiser for increasing community access to midwifery care and rescinding all rights to compensation for images or text. I hereby affirm that I am over the age of minority and have the right to contract in my own name. If signing for a minor, I hereby state that I am the lawful parent or guardian for the minor. I have read the above authorization and fully understand the contents. Each participant name above confirms participant consent for the photo project release.
